Appointments

We look forward to collaborating with you in your care. We welcome both established and new patients to our practice. We participate with most insurance plans (see below). If you are a new patient, please contact our office in advance to schedule your appointment. Existing patients may call, email, or send us a request via our secure Patient Portal. We will do our best to make an appointment for you in a timely fashion.

Call (910) 904-1695 during office hours to schedule your appointment. Our receptionist will ask you questions so we can schedule the appropriate type of visit. Please note that, given insurance company constraints, problem visits need to be scheduled as separate visits from annual preventive exam visits. We dislike this as much as you do, but insurance companies tie our hands on this.

Patient Portal

Existing patients can log in and request an appointment using our convenient Patient Portal, as well as review their health information and send non-urgent questions and refill requests. Communications through the portal are confidential and secure.

LOG-IN

Please arrive 15 minutes before your appointment time to complete any paperwork.

If needed, download this Disclosure of Confidential Health Information CFR45 form, fill it out and bring with you to your appointment.

Cancellations

If you are unable to keep your appointment, kindly call the office at least 48 hours in advance to cancel or reschedule so that others may use the time we had reserved for you.

If your insurance is not on this list, please call our Billing Office (910) 904-1695 to arrange courtesy billing to your private insurance. If you do not have health insurance, please also call our Billing Office if you have questions about billing and payment.
